You do not have to pay a large private fee to get addiction treatment in Japan. Treatment for alcohol and drug dependence through Japan’s statutory health system carries only the usual insurance co-payment, and peer support such as Danshukai, DARC and Alcoholics Anonymous or Narcotics Anonymous meetings is free or low-cost. Private residential rehab, where it is used, is commonly advertised by private rehab clinics in published international directories from around £8,000 to £30,000 a month, and treatment abroad can sometimes cost less than an international private stay. This page sets out the real cost of rehab, option by option, so you are not left wondering how much rehab costs before you decide, whether the dependence is on alcohol or drug.

Getting help in Japan: the free and low-cost options
Start here. These addiction treatment and rehab services are public, national or peer-run, and for a great many people they are the right first step, whatever the drug and alcohol issue you are facing. Cost is rarely the barrier to beginning.

Who each option is for
Because addiction affects both mental and physical health, including psychological addiction as well as physical dependence, getting a same-week appointment at a Mental Health and Welfare Centre is usually possible. Mental Health and Welfare Centres and Public Health Centres are the public starting points: they give confidential consultation, advice for families, and referral into drug addiction treatment or care for alcohol dependence, including inpatient psychiatric hospital care where that is needed. The Yorisoi Hotline and TELL Lifeline are for anyone who wants to talk first, with English support available. Danshukai focuses on alcohol and is a Japan-founded fellowship for people living with alcoholism, built around a daily promise of abstinence, while DARC focuses on drug treatment through peer-led recovery homes developed in Japan, where people in recovery live and work through a structured day together on a model close to Narcotics Anonymous. AA and NA hold free meetings, including English-speaking ones in the larger cities and online, and sit alongside any formal treatment plan. Whether it is drug or alcohol addiction, heroin addiction or dependence on another illicit drug, the referral route into these addiction treatment services is the same.
Private residential rehab in Japan
Dedicated private residential rehab for addiction, in the Western all-inclusive sense, is limited in Japan. Most structured inpatient rehab happens either through admission to a psychiatric hospital with a dependence service, funded mainly by statutory insurance, or through a peer-run DARC recovery home; outpatient rehab and outpatient treatment are more widely available than residential beds. An English-speaking drug and alcohol rehab, private alcohol rehab centre, or drug rehab clinic of the kind common in the UK, Australia or the United States is scarce, and none could be verified as genuinely there for this guide. If that is what type of rehab you are looking for, the honest routes are a psychiatric hospital referral through the public services above, a DARC home, or a private residential rehab programme in another country.
What treatment costs in Japan
How you pay for rehab in Japan depends heavily on the route you take.
Through the public system. Japan’s statutory health insurance covers medical rehab treatment for alcohol and drug dependence, including psychiatric hospital care, alcohol detox and therapies such as cognitive behavioural therapy, and patients typically pay a share of the cost, generally around 30%. For eligible outpatients receiving outpatient treatment, the Autonomy Support Medical Care scheme (jiritsu shien iryo) can reduce that share further. This means a hospital-based route is far from free, but it is a long way from full private fees.
Peer and community support. DARC recovery homes charge modest monthly fees for drug rehab and drug treatment, which are sometimes supported by public welfare for those who qualify. Danshukai, AA and NA ask only for a small voluntary donation for ongoing alcohol rehab and peer support, and the helplines are free. The cost of alcohol rehab through these peer routes follows the same low-cost pattern as alcohol and drug rehab generally, wherever you are in the country.
Private and international residential rehab. Where private treatment for alcohol addiction or drug addiction is used, whether the few residential rehabs available in the region or a programme abroad, published international directories commonly advertise fees from around £8,000 to £30,000 a month. The difference in cost between providers is wide: these are indicative ranges of what private rehab typically costs, compiled from each provider’s own published figures, not quotations for any named clinic.

Which option is right for you
There are treatment options at every price point, from free to private, and the type of treatment that suits you depends on your situation. Start with a Mental Health and Welfare Centre or Public Health Centre if you are in Japan, unsure where to begin, or want to stay close to home, work and family. Use Danshukai, DARC, AA or NA alongside whatever else you do for ongoing substance use support and relapse prevention, they are free or low-cost and available across the country. Ask about psychiatric hospital care if you need drug addiction treatment or a supervised detox and want to use your insurance. Consider treatment abroad only if you want a private, English-speaking residential rehab programme with structured aftercare that is hard to find locally, distance from your usual surroundings would help, you can travel, and you do not need an immediate hospital detox first.
